  • College Career Day Form -  Please return completed forms, with signatures, to the Guidance Office at least 5 days
    before your scheduled visit. You will need to bring back a statement on letterhead
    verifying that you were visiting on that date.
  • Prearranged Absence Form - Please complete the attached form if your will be absent due to a family trip or other for another non-medical need. 
  • Senior Brag Sheet - Seniors, use this form when requesting a letter of recommendation from a teacher,
    coach, etc. 
  • Voluntary Drug Testing Form- You must submit this form if you participate in athletics, school clubs, band, orchestra, choir, or you are a student driver. 
  • Work Permit - MHS students - complete all parts of form and submit to the secretary in the Welcome Center.
                                PHMS Students - complete all parts and submit to the secretary in the Guidance department.
